Remuneration Value: $31.09 - $34.08 p/h (FTE $61,438 - 67,347) + 9.5% Super + Salary Packaging

BlueCare is currently looking for Lead Practitioners (Service Design) to join our community care team based in Charleville

Your community:

Immortalised in Slim Dusty's song by the same name, Charleville is the largest town in Queensland's south west. In the heart of 'mulga country', Charleville and surrounding pastoral properties are rich in history, flora and fauna.

Charleville is home to south west Queensland's largest Royal Flying Doctor Service base that serves remote communities as far afield as Birdsville, 900 kilometres to the west.

Your role:

In this role you will act as the lead disability practitioner at a service level providing guidance and leadership to employees delivering disability services. In addition you will work closely with individuals and their families throughout the NDIS planning process to agree on support plans and convert those plans into service agreements and service bookings to allow individualised billing consistent with NDIS pricing schedules.

You will have excellent leadership capabilities with the ability to manage multiple priorities and stakeholders, along with highly developed interpersonal communication skills to assist you in building rapport with the person we support, their family and their support teams. In addition you will have experience with service delivery and redesign at an individual and group level preferably within disability.

You'll demonstrate
  • Intermediate to experienced systems experience (including MS Office and Care Management Systems)
  • QLD Driver Licence and roadworthy registered vehicle
  • A current Working with Children Check (Blue Card) or willingness to obtain
  • A current positive notice for working with people with a disability (Yellow Card) or willingness to obtain
