We offer the services of Plan Management and Support Coordination to NDIS participants. 
Plan Management makes it easier to manage NDIS funds by freeing our customers from the time-consuming daily admin tasks and keeping them up-to-date on their expenditure. 
Support Coordination with Plan Partners involves working closely with our customers to understand their needs and goals, connect them with people and services who can contribute meaningfully to achieve unique goals as we help navigate through the complexities of the NDIS.

Your Role ...
As a Support Coordinator, you'll work with our customers, their families and carers to coordinate all aspects of their NDIS plan and help them to get the most out of their NDIS journey. You'll find and connect them with service providers in their area that are best suited to their situation. You'll help them understand what's in their NDIS plan, set up service agreements with their providers, report outcomes to the NDIS, prepare their plan reviews and support them when issues arise with their service providers.
 
As Plan Partners is completely independent and doesn't provide any other services than Support Coordination and Plan Management, you'll be sure to work in the interest of your customers only. Your main focus is to build their skills and confidence and ensure that the goals in their plan are being met. Backed by a dedicated team of experienced Support Coordinators with many years of experience in the disability sector and a deep knowledge of the NDIS, you'll have the opportunity to grow your skills and expertise, and play a key role in the lives of people with a disability.

Success is...

  • A minimum of 4 years' experience within the disability sector
  • A Certificate IV in Disability, or another such Human Services qualification
  • Understanding of the NDIS and associated legislation
  • Current Police and Working with Children's check
  • A valid drivers' licence, good driving record and reliable car
  • Ability to self-motivate to meet expectations, excellent time management
  • Ability to work successfully autonomously
