CatholicCare Ageing, Dementia and Disability Care focuses primarily on the delivery of a range of services to frail older people, people with dementia, or people with a disability, as well as their carers.
Their services are tailored to individual needs, support client choice, access to community resources and independent living. CatholicCare offers affordable services that are responsive to the cultural, religious and spiritual context of the client. Their programs provide support to clients experiencing varying levels of emotional, physical and intellectual disability.
Services are available throughout South West, South East and Inner West areas of Sydney.
CatholicCare Ageing, Dementia and Disability Care's ideals are to provide continuity in care, prioritised access and service flexibility through a client centred, strengths based model of care.
Each client receives a holistic assessment and culturally sensitive information to assist with making an informed decision about the type and nature of care best suited to their needs. An individual care plan is developed with each client with input from family, carers, advocates and other relevant service providers.
